Lyra Health, Modern Health and Spring Health are the three largest EAP providers in the United States. Between them, they offer mental health sessions to thousands of employers. Click on each one to explore if your company is partnered with them. 

HOW TO FIND OUT IF YOUR COMPANY HAS AN EAP

Check your company's benefits portal or intranet.

Look for information about mental health services or employee assistance programs (EAPs). 

Contact your HR department.

Ask them about your company's mental health benefits and if an EAP is part of the program. 

Look for information in your benefits materials.

 

Your company may have provided information about EAP benefits in onboarding materials or employee handbooks. 

​If you still have questions about EAP coverage or your company's benefits, contact your HR department for clarification. 

Out of Network Benefits

Even if your insurance company is out-of-network (OON), you can still receive therapy from Interval Health without breaking the bank. 

 

Many health insurance plans (especially PPOs) include OON mental health benefits, meaning your insurance helps to cover the cost of therapy even if your preferred provider is OON. 

Depending on your specific plan, your insurance carrier may reimburse you anywhere from 50-90% after you meet your OON deductible for the year - making therapy with an OON therapist more affordable than you might initially think.

SUBMIT SUPERBILL TO INSURANCE

We provide you with a monthly superbill (a receipt of the therapy services you have already paid for) with all of the necessary information for you to submit the claim to your insurance.

GET REIMBURSED

Upon processing the claim, your insurance will reimburse you for a portion of your therapy session (dependent on your plan), typically in the form of a mailed check or direct deposit.

QUESTIONS FOR YOUR INSURANCE COMPANY

 

When contacting your insurance provider to determine the details regarding your out-of-network benefits for mental health services, here are a few things you will want to ask:

  • Do I have a deductible? If so, how much is remaining?

  • How much will be covered per session?

  • Do I need to obtain a prior authorization?

  • Is there a limit to the number of sessions per year?

  • How do I submit receipts for reimbursement?​

PAYMENT
For telehealth sessions, all major credit cards are accepted. Payment is due at the time services are rendered. FSA & HSA cards are also accepted, allowing you can access care with all the tax benefits. Or you can submit receipts to your FSA provider for reimbursement yourself as you would with any other medical service. 

CANCELLATION POLICY
If you are unable to attend a session, please make sure to cancel at least 24 hours in advance. Otherwise, you may be charged a $75 late cancellation fee. 

Paperwork/Non-Clinical Fees

If you require paperwork to be completed for a third-party source (such as Medical Leave of Absence or FMLA forms), there is a flat $75 fee which must be paid in advance of the paperwork being completed. 

Other professional services such as report writing, telephone conversations lasting longer than 15 minutes, attendance at meetings with other professionals you have authorized and preparation of treatment summaries will incur a fee of $200/hour, prorated to 15 minute increments. 

 

If you become involved in legal proceedings that require your therapist’s participation, you will be expected to pay for any professional time spent on your legal matter, even if the request comes from another party. Please note that these services are not reimbursable by insurance.
